Passport Seva Kendra (PSK) - to Apply Online for Fresh Indian passport or the Re-issue of passport
Applying for fresh passports or renwal of passports are very easy. Govt of India has lots of PASSPORT SEVA KENDRA's (PSK) established all over the country and you can Apply online for passports or renewals in few simple steps.
For this You have to logon to Passport seva kendra's website
http://passportindia.gov.in
To use the website, you must have a valid User Id. If you are new to website, then Create a new User Id

Creating new user:
Click on "New User Register" button.
In the form enter your passport office and all reuired details, along with USER ID and PASSWORD (You can user your email address as User id)
On submission, an email will be sent to your registered Email Inbox
Click on activate link in the email and confirm activation
The login using your new User ID and password

If you are an existing user , Use your Login Id and Password to login to the PSK's portal
The basic documents required for registration are
Birth Certificate - (You can use your SSLC / Marticulation certificate )
Address Proof - ( You can use your Driving licence or any other valid proofs)
After login, in the Applicant Home page you will see following links
Click on "Apply for Fresh Passport/Reissue of Passport"
On the resulting page You will be provided with two alternatives to apply for a fresh passport or Reissue of a passport

Alternative 1
You can download the soft copy of the form. Fill it offline (internet connection is not required to fill the form offline). After submission an XML file is generated, which is to be uploaded back to the PSk's site.

Alternative 2
You can apply online. Click on the link "Click here to fill the application form online" to apply online

On Next page enter
What you are Applying for : Fresh Passport or Re-issue of Passport
Type of Application : Normal(takes atleast 45 days) or Tatkaal (20 days)
Type of Passport Booklet : 36 Pages or 60 Pages
and on other pages enter the required info.
Select the passport seva kendra office and choose a date for appointment for verification of orginal documents.
Submit your application form and take the Print out of your online application.
On the print there will be your appointment details. Be at the PSK on the date and time of your appointment. After the formalities there, there will be enquiries about your claims also there will be police verification.
If everything goes well, Finally Your Passport will be mailed to your Address.
Please note: There will be a file number for your application (Which is given during your appointment at PSK). Use this File Number for tracking the status of your application.
